Insight Manager - Custom Commands Blank/Empty

We are on Insight Manager 5.0 SP3 and I had entered several custom commands and setup some automatic event handling tasks that point to these. Somewhere in the last few days all of the custom commands do not show up. I tried adding one of them back and it says it already exists. I added it with another name and it took it but does not show up in the list (still no entries). I have cycled the Insight Manager Server a few times but they still do not show up. Any ideas?
