Migration of SIM 4.2 to a New Server

I need to migrate my current SIM 4.2 server to a new server. I am currently running on SQL 2000, and Windows 2003. I have found references to a migration tool, but have not been able to find the tool. Does anyone know of any instructions and/or tools that will assist me?
