Re: cannot open SAN devices
Does ioscan show hardware?
Can you run diskinfo on the raw files?
Some disk arrays will show the device on a path but will not allow access. Typically the path is on the non-active controller. I've seen this on the EVA and Clariion.
Is security on the LUN set so that the HBA has read and write access to the LUN?
Are you using the correct tape drive? (silly question, but I've seen confusion with multiple tape drives, tape mounted in drive 1 and device file seleced is for drive 2)

Re: cannot open SAN devices
The only problems that patches fixed for me was not seeing the devices and building the device files. Once I got past that, I never ran into any other problems.

Re: cannot open SAN devices
First I removed the devices, rmsf -H 0/X/0 and all associated devices. some Devices you may have to "rm /dev/rmt/xxxxBEST0m" etc.
ioscan
insf
This creates the devices.
Then we reconfigure in netbackup. That is fine until someone changes the san...
A SAN/TAN fibre change forces us to reconfig completely.

Re: cannot open SAN devices
e.g., insf -e
So, if insf doesn't work try it with the -e
