may use the AC (Alert Display Configuration) command.
Moreover earlier FAULT LED was ON and current status is also FLAshing which means there is a hardware fault.
Have a call logged with HPRC.

but something went wrong:
ENTRY_INIT Status = -7
Do you have an internal DVD drive and just attempt to boot from the wrong device by chance?

Now you try to boot from tape - is this made by IGNITE? I think so.
Where it is taken from?
What version?

Can you be more specific you are trying to install from what media? are you trying to restore or do a cold installation?

Here is the error code means
ENTRY_INIT -7 Causes:
- Booting from the wrong path or non-existent device.
- Trying to boot from a device that is not responding.
Troubleshooting Steps:
1) Power off the server, then power off the disks - leave all powered
off for at least 30 seconds. Power on disks, then server.
2) When server is booting, check access LED's on boot disk, is it being
accessed? Yes - possible bad disk. No - possible bad path to
disk, e.g.. Cabling/termination/adapter. Time to log hardware call.
3) If boot disk is in an external disk enclosure, check the cabling is
secure, and access/status lights are normal.
4) Attempt to boot from an alternate boot path if you have one,
e.g.. Mirror boot disk, alternate boot disk.
5). Double check you have the correct boot path.
6). Use the SEArch option at the Boot Menu, does the primary boot path
appear? If not, we cannot see the disk and time to log a hardware call.
If we can see the boot disk, but still get ENTRY_INIT error when booting
we need to log a hardware call.
7). We can boot to Recovery Shell and use the chroot option to fsck the
boot disk, but this will always fail if we cannot get to the ISL
prompt of a disk; in most occurrences of this error using the Recovery
Shell (Support Media) is not helpful.
8). Perform preliminary steps to reinstall the system; As in most cases this
error means the boot disk is bad and will need to be replaced. If the
system is not mirrored then the recommended recovery method is to
reinstall the system. Is the backup offsite? Do you know what sizes
the vg00 logical volumes are?

1)You are booting from 0/0/1/0.5 which is an external tape drive.
2)You are using an ingnite tape.
3)The INSTALL kernel is loading and as soon as it finishes loading there is a panic.
4)Since there is no dump device yet (and there will never be) there is no panic dump.
5) The hardware resets and starts over.
I am guessing that you either have a flaky hardware, or the INSTALL kernel you are using in the ignite tape came from incompatible hardware.

I have new disk and trying to install OS from external DVD drive and still have errors...can't continue installation on L2000 server..ataached are the errors i've got

The SEA command did not report any devices other than your installation device - did you interrupt the search or did it not find any disks?
If the system cannot see any disks, installing the OS will obviously be impossible as the system won't see any disk to install *to*. If your system in fact has disks installed, you have a hardware failure that prevents the system from communicating with the disks. At a guess, the fault might be located somewhere in the I/O backplane or the disk backplane.
Your PDC firmware version is 40.48: this is very old, but apparently just barely new enough for 11.11.
The latest firmware is 44.28, available here:

Open a hardware support call with HP and send these logs in for analysis.
