ioctl error while backup

I am getting the following error while taking backup.

fbackup(3044): ioctl error, can't query outfile
at media record 3786
fbackup(3102): attempting to make this volume salvagable
fbackup(3123): could not read the previous checkpoint record
fbackup(3105): writing 2 EOFs and rewinding the tape
fbackup(3106): please mount a good tape
fbackup(3038): output file /dev/rmt/0m is not a valid device file for backup
fbackup(3019): would you like to enter a new output file?
fbackup(3004): writer aborting
fbackup(1002): Backup did not complete : Reader or Writer process exit


I tried changing new media,cleaning the drive. But still i am getting the same error. Last month also i received the same error, as per HP forum i changed the drive & it started working.

I thing, it's looking for some pacth. Can anyone help me out.

Probably a bad tape.

Suggestions:
1) New tape
2) Cleaner tape before next recording.
3) Both of the above.

If there is only one tape drive, you can remove device file and create again.
#rm -f /dev/rmt/*
#insf -eC tape
Check for the cable. Sometime the cable wasn't tight.
Reboot the server if can...
