It would help if you told us what make and model of Fibre Channel Adapter (FCA) you are using, and what SAN storage (MSA 1000, EVA 5000, XP 12000) you are trying to boot from.

EVA5000 support HP branded HBAs and few amount of native HBAs. HBAs from qlogic mentioned in this document and supported with EVA5k are FCA2214 and QLA2340/2342.
